Return Policy

The Provider Lifestyle family wants you to love what you wear. If you are not pleased with your purchase, we will process an exchange or refund within the following terms.

We are happy to process your return or exchange on regular price merchandise within 30 days of purchase date.
Please be sure your merchandise is in its original condition with original tags attached, and it is unworn and unwashed.
Make sure you have a return form prior to returning your merchandise to us. Customers can request a return form by email (info@ProviderDesign.com). In the subject line please put RETURN REQUEST FORM

Once your return form has been issued, merchandise must be received within 7 business days at Provider Lifestyle, so don’t forget to ship it back.

As a friendly reminder, our shipping and handling fees are non-refundable. If an item is damaged or defective, the return shipping is on us! Please send photo examples of this when you request your return form.
Temporary or special promotional sales qualify as regular price merchandise – let us know if you have any questions.
Promotions and gift cards cannot be returned or redeemed for cash value.
Final Sale items cannot be returned or exchanged. Items are considered Final Sale when discounted 50% or more before using any promotional code.

Refund Notes:
Our refunds will be issued in the original form of payment and require 4-5 business days to process. Thank you for your patience.
Please note, we can process your refund only once we have received the returned merchandise back.
We will give you a shout via email when the return has been processed and a credit has been issued in the original form of payment.

Exchange Notes:
Our exchanges require 3-5 business days to process. Thank you for your patience.
We will give you a shout via email when the exchange has been processed and the updated items have shipped.

Return Mail Requests
Please make sure merchandise is securely packed when shipping it back to us.
Please enclose a copy of the original return form. Remember – you have to email us at info@ProviderDesign.com to request a return form.
Another friendly reminder to please prepay your return shipping charges. We cannot accept C.O.D. packages and deliveries.
It is important that packages are shipped through a traceable shipping method, such as UPS or Insured Parcel Post. Make sure to retain the return tracking number for your records. Provider Lifestyle cannot be responsible for lost inbound packages, so we want to make sure your return is received safe and sound.
